First time using an NVMe SSD. I'm installing this in a PCI card on an older PC. Do I need to peel the sticker off before I install? The PCI card comes with a heatsink. Will it affect the SSD if I leave the sticker on?

Peeling it off may void the warranty. Whilst it should improve the thermals somewhat (especially with a heatsink), the difference (single digit deg C under load) is not substantial enough to risk the warranty. AFAIK stickers these days are designed with thermal conductivity in mind.
