How is the $3 off applied? Does Amazon account have to be connected to the Amazon account?

It's applied automatically at checkout, provided you do this:

https://www.google.com/search?q=how+to+connect+Amazon+account+to+ring+discount&oq=how+to+connect+Ama... [google.com]

It only has to be done once, then all future Ring purchases (at Amazon) get the 10% discount. At least, while they maintain the promotion.

Does the 10% apply to all Amazon / Ring [ring.com] purchases?

EDIT: It only applies to certain products listed here: https://ring.com/collections/ring...0-discount

It appears the Indoor Cam is not listed on that list...so 10% may not apply?

Yes you can, but motion detection doesn't work at night time when it's dark outside.
