Yeah, you really have to wonder how all those "stores" that sell these as new on Amazon got their hands on them to sell super cheap. Most of 'em aren't even tool stores, something seems fishy there.
That being said, this is my most favorite power tool and I highly recommend it. So smooth and comparatively quite quiet, it's just a pleasure to use.

Hex refers to the chuck. It will hold 1/4" hex bits.
Hydraulic refers to the impact mechanism. Instead of a hammer and anvil, it uses expansion/contraction of some oil. It's supposed to be quieter and the impulse is a little different.

You can think of this as a dampened impact driver. Less noise but hits softer on the fastener. More of a... well, surge than a hit with each impact/impulse.
While the peak torque is less than a traditional impact driver, the overall work done is about the same. And again it's significantly quieter than an impact driver. Also feels a lot smoother to use. Less vibration back into your hand.

If you have the M12 Fuel is the Surge worth it to have also? I hear it is quieter but has less power. Have you used both?
I don't have mine yet. Just ordered on home depot a couple of days ago. Similar pricing with the current sale there.
It's definitely worth it for me if I don't have to keep wearing hearing protection. There was a review where someone did a decibel comparison and it was noticeably quieter.
If you don't mind the loudness of the impact driver. It's probably not really worth it.
With that being said, everybody loves the surge.
