The quality of life improvements are great such as showtime attacks, ammunition replenishes every battle, and much more than I can list.
They've also reworked mementos, added two new confidants which also really shape up the story.
New third semester adds 20-25 hours of extra gameplay with the new palace being my personal favorite. If you wish to play it, you will have to play through the entire first two semesters first (no way to start the third semester out of the box or transfer old saves)

If you're itching to replay it and want to see extra story, then it is absolutely worth it.

I haven't played that, but I can tell you this is one of the best RPGs ever made. I had never played a Persona game before 5. I ended up going back and playing Persona 4 Golden after this and can't wait for the remake of Persona 3.
I first beat the regular version of this game and did not want it to end, even after I had already put 100 hours into it. Then when they came out with Royal, I paid full price again (because it's worth it), and played through the whole thing again, since they added another 20 hours and a new character who has her own fleshed out story.
This game is just incredible.
