Edit: immediately got the $15 credit email, so this still works, despite the promo code weirdness.
And if you apply it to your account you will see that error if you try to apply it again. This has nothing to do with app vs desktop- as far as Amazon was concerned you had already added the code to your cart somewhere/anywhere, hence "once per account".

Typical inept "customer service" provided by Amazon, involving conveyance of an insincere apology and report of escalation, which results in an additional apology and failure to resolve an issue, and/or misattribution to unrelated factors.
