Using a VPN did not seem to resolve mistake 1, but today, after returning to the US, I was able to link to a dormant P+ account after signing into it and using the OP link. Maybe mistake 2 was not a big deal, but try to look up all your P+ accounts/e-mails before jumping onto SL.
