I just picked this up from a Costco in Phoenix. $32 after tax and the $25 credit for PlayStation can still be claimed.

Costco in Naperville, IL (Ogden location) has this deal as well. Such an amazing deal. Basically $5 for the controller if you don't count the $25 psn gift card.
