Personally I prefer 100% cotton pajamas for my own kids.
I mention it primarily because it should be part of the original post. Those shopping kids pajama may or may not be familiar with the fact that 100% polyester pajamas are generally true to size (so a kid wearing 4T clothes would wear 4T 100% polyester pajamas without problem) whereas 100% cotton pajamas are snug-fit to accomodate regulations[cpsc.gov] around childrens sleepwear (so the best fitting pajama choice can vary, but typically recommends a larger pajama size relative to their current clothing size).

Depends on the brand. For example, Hanna Anderson sizes take into account that the outfit goes into the dryer in low setting.
