So is it 1 dog up to $150 back AND 1 cat up to $150 back max potential reward is $300 per household , or is it one reward per household total max reward $150? Terms and conditions say "The following limits apply: one (1) dog and
one (1) cat reward per household; maximum US $150 total Reward." which is clear is mud.

has anyone else tried this? I uploaded both receipts. I fill in the contact info. I confirm my phone number, I select to be paid via paypal, but then there is literally no 'submit' button. There's no way to get the form submitted. I've tried on different browsers on different PCs. So frustrating.

Hi just went to mess with it (using my iPhone on safari) it required a phone number verification below the phone number entry- it says something like OTP verification. Once I did that and made sure all the boxes were filled in and not yellow from autofill, the submit button finally popped up.

I did not find any mention of what kind of Vet visits does it cover. We could usually do Vaccines, Chip, and Neuter/Spay for $150.

I would love to know this as well. What procedures are already predetermined eligible for the cost reimbursement up to $150.

It's the checkup only. I finally got it to go through. The vet bill was over three hundred dollars for all of the annual stuff they do in connection with the check up, but Iams reimbursed only $75 as the examination fee was listed at $75.

So only two food receipts allowed - it says pre coupon and tax price, so the discounted iams at Amazon right qualifies if you snag a couple at a total under $50 with discount?
