Under Settings > Account > Google Voice number, make sure to 'Unlock'
Account # is your GV phone #
Pin you have to create in your GV account settings by toggling on "Call to listen" under voicemail settings.

Based on info in this thread as well as info found on google, it is recommended to use google's address if you are porting a GV number and are asked for an address when providing port info for your 'old carrier':

Once activated, Text UNLOCK to 611611 (send it twice, it wont work first time) to confirm your unlock date. (approx. 60 days) The unlock is automatic and sent from apple servers. And it will unlock from anywhere in the world.
Bypass port in requirement for activation
YMMV Bypass port requirement After getting device activate on total website at top click Activate>New Phone One you've purchased through Total Wireless>I want a newer number. Plan was tied to phone IMEI on activation.
YMMV, you can activate your phone on tracfone.com with new number directly to bypass port in requirement and the cost is $10 But please be reminded, after you input the imei on tracfone.com, you prepaid balance for your total-by-verizon will be lost and your phone will be rebrand to tracfone and you can't activate it on total-by-verizon wihout rebranding it back to total-by-verizon
You are NOT required to pay for a second month of service!
**Confirmed it doesn't work with a Verizon prepaid SIM card inserted 24 hours after activating the phone on Total VZ sim card.
Chase 15% Offer Details (YMMV / Targeted)
NOTE - this deal is YMMV / Targeted. I have several chase credit cards, but the offer only appears on 2 of the cards.
1) Log into chase.com account
2) Accounts tab -> (top left of screen)
3) On the right hand side of the screen you will see a "Chase offers" panel, click it
4) Top right corner there is a "choose account" drop down. If you have multiple chase cards, you will have to cycle through all of them and check the deals below that appear. not all of them will have the Verizon offer (hence YMMV)
5) If you see "Total by Verizon" deal then click on the "+" to add the offer to your card. NOTE - Remember which card you added it to (i.e the last 4 digits of the card)
When checking out on TotalByVerizon.com, use the Chase credit card that you added the offer to, to pay for the phone.
You will get an email notification a few days later, if successful (Subject "Congrats! You've successfully redeemed one of your Chase Offers")

2. I couldn't pass verification since the numbers bought can't recieve actual text, then people told me I can use any number I have to get the text, I won't be entering the port number until later. Used personal phone number for one, and a GV free number for the other.
3. Recieved phone and activated first one online with the ebay bought numbers no issues. Select keeping your number for the port in, entered my personal address and the info ebay seller sent for account and pin. Second phone wouldn't activate online. Called in and they activated it but also took payment of $30 after verifying I had another line already. Then I realized I already paid for that second line's first month when I ordered the phone. Refund was a bit messy, eventually they refunded it.
4. Put sim in to start the activation on Total and marked the phone for 60 days unlock. Put away in drawer.
They asked for enabling auto pay say no. They asked to join rewards program sure why not.

Edit: Now that I think about it, there is no way to tell. Just check sickw in 60 days and it should show unlocked.
