I saw some NHTSA investigation on these but seems to be attributed to user error. The NHTSA doesn't seem to mention any details, which seems to imply there were no injuries, and seems to imply user error. After browsing and seeing how violently opposed many parents are to reading, EVEN THE STICKER ON THE SEAT, I am convinced that is the cause.
On the positive side this seems to have one of the largest rear facing limits at 48" and 50 lbs.
