Everything I found indicates that an explosion is almost 100% a defective pan that had air pockets or some other impurity in it. The most dangerous thing that can "normally" happen which is still loud is thermal shock causing the pan to crack usually in half or along fracture points where the shock occurred. Even this is not at all common with cast iron as millions of people take their pans straight off the stove and run cold water on them in the sink or throw them on an already hot burner then drop frozen stuff straight into them but doing things like that can cause thermal shock and result in the pan cracking it's just not very common. An actual explosion of a pan with pieces everywhere is almost guaranteed to be a defective pan and I've never even personally seen it happen or heard of it happening to anyone I know despite some of them having thrown thrift store cast iron in fires overnight for decades.
The only thing I found that makes it somewhat more common is people using the enameled cast iron options as apparently it's more common for there to be air or liquid trapped between the cast iron and enamel layer which can cause it to explode easier than cast iron by itself.

America's test kitchen did some reviews though of cast iron pans and there were a couple new brands with smooth finishes that were easier to season and were more non-stick consistently.
Carbon steel is also a viable option.
I've never heard of a cast iron pan exploding. Maybe it had a hairline fracture that caused it to break due to heat stress? I've punished my cast iron (le creuset - factory stores are great) and it has zero signs of issues.
*Edit likely what others are saying, it had some kind of defect.

I'm betting it was Ozark Trail brand, which is made in China.
Their brand graphics are orange & black.
It cracking with a loud 'explosion' noise doesn't happen often, but it does happen on rare occasion. It happened to me long ago with a Lodge 12" dutch oven with the 3 legs and the deep rimmed lid to hold coals. I had about 2 1/2" of cooking oil in it at the time.
Mine was actually caused by a slightly faulty casting running near one of the 3 short legs and uneven heat expansion between the heavier/solid leg versus the regular cookware thickness around it. Plus I was cooking over coals.
Anyhow, it's rare that it happens. I've never seen it happen on any regular Lodge skillet.
