I don't think audio gets plumbed through the adapter itself so it's probably the delay in your head units Bluetooth system. I could be wrong though

Another point is, when I disconnect everything (wired, wireless CarPlay) and use plain Bluetooth for songs and video using my phone. There is no delay in
1. Changing tracks (it happens instantly)
2. No out of sync video-audio.

There might be an input delay in the Bluetooth or some kind of buffering going on, but it isn't latency specifically.
Latency would be if you were playing video and the audio was out of sync.

Moreover, the Bluetooth protocols used for input (changing tracks) is different than the one for outputting audio.
Try connecting a Bluetooth keyboard to your device and see if it exhibits similar delays with input.
