your first and last name, complete mailing address (no PO Boxes), daytime phone number, date of birth, and email address on a 3" x 5" index card or piece of paper and place your entry into an outer-mailing envelope with proper postage affixed and mail it to: 2025 Club Applebee's NFL Digital Game Play Request, c/o GSE, PO Box 160, Dept. 25-8129-01, Glencoe, MN 55336-0160 to be received by December 31, 2025.
TIMING: The Game begins on October 6, 2025 at 8:00:00 a.m. Eastern Time ("ET") and ends on January 4, 2026 at 11:59:59 p.m. ET or when all Game Plays
ELIGIBILITY: The Game is open only to legal U.S. residents physically residing in the fifty (50) United States or the District of Columbia who are eighteen (18) years of age or older (nineteen (19) years of age or older if a resident of Alabama or Nebraska) and a member of Club Applebee's®, at the time of entry.
