Does it say it is already clipped by any chance? They wouldn't feature it in the ad like that if it were account specific. very strange if so.

that's what my screenshot shows, shows $10 Wags Cash on $45 on the envelope. that IS the bonus offer. the other two offers are on the Oral B items ($15 on $50 and $15 on $45) all three add up to $40.

be sure you've added the 6 count refills and not the 3 count. refills are about $34, not $28! That's likely the issue. and don't apply any wag's cash either.
