Even though the FDA has officially approved Blood Pressure and ECG monitoring for US users, they approved the US-specific version of the hardware and software. When Samsung's system sees a non-US watch operating in US territory, it flags a compliance error and immediately locks those specific medical features down.

It's a great watch: better battery than my regular Watch 8 and the rotating bezel is a really nice feature that actually a lot of benefit beyond the standard digital rotating bezel.
One thing worth noting: I believe the international version has some health features that are restricted in the US vs the US version where they are all unlocked. That said, while it sounds nice to have all of the features, realistically 95% of people are not using every single health feature anyway.
