You DO NOT need to enter in ANY credit card info or sign up for autopay. Leave that part BLANK. Just put any name, address, phone and zip code, check the boxes and click activate.
Is this a $5 sim kit + $25/mo cell plan?

The product description on Amazon shows as "First Month Service - Included" but that seems too good to be true
Network you need to check it on
At&t boost website will tell you it can be activated when it and none of customer support
Will tell you or fix your problem so check your phone first at att site
