Sony doesn't allow purchasing systems with the gift cards, only digital game purchases and PS plus memberships

It's not true. Internet blew up over nothing. Before the change, Sony would grant unlimited license to any digital purchases. So scammers were taking advantage of digital purchases by downloading the game and requesting a refund right away. So when Sony gave them the refund, they kept the game because it was already granted unlimited license.
The new process is Sony grants a temporary license up to 30 days after purchase. When the 30 day refund period expires, the temporary license changes to unlimited license. The timing varies, but if you connect your PS5 at least 15-30+ days post purchase of that digital game, then it should change to unlimited license. You won't lose your purchase. Sony basically added an additional step to prevent fraud.

I've been ordering gift cards online from Costco for the last decade, and there is no rhyme or reason why it gets cancelled. I end up having to call customer service at that point if I really want to make the purchase.

The last time I partook in this deal, my orders were cancelled right after making them as well. When I called CostCo, they explained that you can't make digital purchases online before physically going into a CostCo to get your picture taken/card issued. It's a fraud prevention thing for people who try to make multiple accounts and buy more than the limit.

If you recently bought your membership online and have not yet physically been to a store, that may be why your order was cancelled.

Is it your first online gift card order? A while back, my first GC order got cancelled and I had to talk to Costco chat - definitely has to do with fraud prevention. Sometimes I still get cancellations for online GCs but it's when my delivery address is different than my membership address. When I make it match my membership address, I can reorder and it's fine.
