Make sure to use the AMEX directly on Samsung's site and not PayPal. I made that mistake on Ray-Ban and lost my $35 AMEX offer.

Looks like from your receipt you are not getting the fold reserve credit only the watch reserve credit. I can duplicate this also with out the fold reserve credit. I just did a chat and they said there is an issue in some accounts with the device credit. They sent me a ticket likely won't be resolved for a few days.
