Garmin inReach Messenger Plus is a rugged satellite communicator built for backcountry emergencies, with Iridium global coverage, strong antenna performance, and the ability to send tracking, check in messages, text, audio, and photos. Battery life is described as lasting days or weeks, and it can keep transmitting while moving instead of requiring careful phone pointing. The main caveat is value, since the Plus may not justify its higher price or subscription cost versus the regular Messenger. Satellite links can also be unreliable under trees or without clear line of sight, so it fits best as a dedicated safety tool rather than a full replacement for other navigation gear.

Are these relevant now that iPhones have satellite messaging?
This guy comments the same thing on all the Garmin in-reach threads and gets an answer each time, but still wonders anew every time there's a deal.

Garmin gps watch, even a cheap used one, with offline topo maps would be the first item to add to a kit imo. Easy navigation back to starting point. You can do it with some smartphone apps with offline gps and offline maps, but battery runs out faster on a phone (less than a day) vs a good Garmin watch (days).Add any Iridium-compatible satellite communicator for times when you're deep into the woods and potentially need emergemcy extract. Or think you're gonna get yourself into a bolder trapping your hand situation like that one guy, fall off slopes, etc even though near civilisation. But nothing will save you underground, like caving. Signals won't get out.....Aside from that, thermal protection + water - even a small packable emergency aluminum blanket + lots of water (backpack water bladders, or bottles) + headlamp with strobe mode + leatherman tool at minimum. Then add first aid kit, food, fire sources as needed.
Any recommendation on the used watch?

If you're on a budget and don't need the messaging, consider used Garmin eTrex Vista units. They are under $50 and have a color screen and take AA batteries.

I'm looking to get this unless someone else suggests something better, its for my elderly parents so user friendly is a must. I don't think they will be keen on using a smart watch so i think this is a better fit for them. They do constantly use their android phones however.

My parents live in a very rural area. Only internet they have is Starlink, even cell phones don't work unless on wifi assisted calling/texting. They have a large battery backup (as well as solar panel hookup) that they can just plug the Starlink into during power outages.

Great! Communication covered during emergencies! . . NOPE highest flooding we've ever seen takes out the in rock cemented pole along with the starlink. Had no way to get a hold of them to make sure everything is ok, just drive down and hope roads to the area are still crossable.

You are not going to be getting days of charge with a Garmin watch with GPS enabled the entire time.

iPhone is hit or miss and you need line of sight. Doesn't work under trees. Then you have to keep doing figure 8 motion to connect with satellite.
