What the primary-prevention evidence shows (general adults)
- No mortality benefit. Pooled across 9 RCTs (n≈51,550), multivitamins were not associated with lower all-cause mortality (OR 0.94, 95% CI 0.87–1.01). The largest trial, COSMOS (n=21,442), found no difference (HR 0.93, 95% CI 0.81–1.08). [1-2]
- No cardiovascular benefit. No association with cardiovascular mortality (OR 0.94, 95% CI 0.83–1.06). [2]
- A small, uncertain cancer signal. A modest reduction in cancer incidence was seen (OR 0.93, 95% CI 0.87–0.99), but absolute risk differences were tiny (−0.2% to −1.2%), the effect on cancer mortality was not significant, and the incidence and mortality findings were discordant — limiting confidence. - Bottom line from USPSTF (2022): Evidence is insufficient to recommend for or against multivitamins for preventing cardiovascular disease or cancer (I statement). The Task Force explicitly recommends against beta-carotene (harm — increased lung cancer/CV mortality) and vitamin E (no benefit). [2]
