open to legal residents of the United States of America and the District of Columbia who are 18 or older at the date of entry.
begins on May 17, 2013 at 9:00 a.m. Eastern Time ("ET") and ends on June 30, 2013 at 11:59 p.m. ET
Limit three (3) entries per email address per day for the duration of the Promotion Period.
PRIZES / APPROXIMATE RETAIL VALUES ("ARV") / ODDS OF WINNING: Five Hundred (500) Mayhem T-shirts. ARV of each T-shirt: $20. A total of Two thousand five hundred (2,500) Mayhem Ringtones - choice of two "Mayhem Ring" or "Mommy" - for the promotion duration May 17,2013 thru June 30, 2013. T-shirts will be mailed to winners at the address provided by the entrant's after they are notified of being a potential winner. Ringtones will be awarded via URL link to the Myxer site page providing ringtone download instructions. If winner is unable to accept a prize as stated, prize will be forfeited in its entirety. Limit one (1) prize per email address. Odds of winning a Prize depend upon the number of eligible entries received and the time in which they are received for a given Day. Total ARV of all prizes: $10,000. Allow 4-6 weeks after winner verification to receive prize.
