You do not have to use it all at once. I spent 99 cents of the $20 credit. When I add an item to my cart and begin checkout, it shows on the side that my "Dash Wand Promotion" credit is $19.01.

If you get the Fresh free trial but don't really want it (like me), be sure to go to your Prime/Fresh account settings and select "Do not continue". With that you still keep your 90-day free trial...

Go to amazon.com/alexa and log in. Go to Settings and look under devices. Click on the device name and part of the info provided is the serial# and the MAC address.
