Yep. The wording implies that the listed models will qualify for the full credit, while others will get some lower credit. Could have been prevented by adding the word "only."

After you complete in the trade in via mytmobile.com, there should be a link to print out the mailing slip and instructions on what to do. It's under phone and then trade in status I belive.
