Had to wait a month after the mailing date to request a re-issue. Called today, and they confirmed that the GC had not be used. A new rebate is now being processed. Was told another 2-4 weeks.

Same thing happened to me. I guess next time it's better to ignore the apartment field on the rebate form and simply put the apartment # in the address field as usual.
