Sweepstakes begins 12:00 PM Eastern Time ("ET") June 20, 2011 and ends 11:59 AM ET July 18, 2011.
Open only to legal residents of the 50 United States and the District of Columbia who are 18 years of age or older and who have reached the age of majority in their state of residence at the time of entry.
During the Sweepstakes Period, you may enter the Sweepstakes in three (3) ways:
(i) go to the "Mom's Day Off" Sweepstakes application at http://www.facebook.co
(ii) play the Crazy Day Pinball Game, submit any score above zero (0) and complete and submit the official entry form as described above. Limit one (1) entry per person, per household and per e-mail address per day via this method.
Up to twenty-five (25) additional Sweepstakes entries may be received by referring friends to enter the Sweepstakes as follows: After successfully submitting your entry, share the unique URL provided on the "Thank You" web page. For each valid participant who enters the Sweepstakes online via the unique URL you shared, you will earn one (1) additional entry into the Sweepstakes (up to a maximum of twenty-five (25) additional entries).
(iii) to obtain entries without completing the online entry form, playing the pinball game, or referring friends, hand-print on a 3" x 5" card your first and last name, date of birth, street address, city, state, zip code, phone number (including area code) and e-mail address (optional) and send in a stamped #10 envelope with proper postage affixed to: Samsung "Mom's Day Off" Sweepstakes, P.O. Box 982, Gainesville, VA 20156. Limit one (1) mail-in entry per envelope per person and per day as determined by the postmark date on the envelope. All mail-in entries must be postmarked by July 18, 2011 and received by July 25, 2011 to be eligible. The first mail-in entry received will count as twenty-seven (27) entries. Each subsequent mail-in entry will count as two (2) additional entries. An entrant may receive up to eighty-three (83) entries via this method, which is the maximum number of entries attainable during the Sweepstakes Period per person.
Limit eighty-three (83) entries per person, per household and per e-mail address for the duration of the Sweepstakes Period, regardless of method of entry.
Grand Prize Package: One (1) Samsung 4 Door LCD Refrigerator (Model RF4289HARS) (Estimated Retail Value ("ERV") $3,499.00) and one (1) Spa Day for two (2) (awarded as two $300.00 SpaFinder Gift Certificates) (certain terms & conditions as specified by issuer apply) (ERV $600.00).
Total Maximum ERV of Grand Prize Package is $4,399.00. Prize includes costs of delivery and installation of appliance. If installation is not available through winner's local retailer, separate installation will be provided up to $300.00 in value. Any costs of installation in excess of $300.00 will be the sole responsibility of the winner. Installation will be conducted by a local retailer selected by Sponsor and will connect new appliance to existing water service only. Refrigerator dimensions with hinges and handles: 35 3/4" (W) x 69 7/8" (H) x 36 1/4" (D).
First Prizes (10): One (1) Spa Day (awarded as a $300.00 SpaFinder Gift Certificate) (certain terms & conditions as specified by issuer apply) (ERV $300.00 each).
Second Prizes (50): One (1) Manicure/Pedicure (awarded as a $50.00 SpaFinder Gift Certificate) (certain terms & conditions as specified by issuer apply) (ERV $50.00 each).
Winners will be responsible for all taxes (including federal, state and local taxes) and any other costs and expenses, including any removal of old appliance (as applicable), any Wi-Fi equipment, set-up and configuration (as applicable) or gratuities, associated with acceptance and use of the prizes not described herein as being awarded. Total ERV of all prizes combined is $9,899.00. Limit one prize per person, per household and per e-mail address for the duration of the Sweepstakes Period.
