It doesn't actually say how many times you can enter. So one entry would be safer than entering multiple times.
There are three ways to enter: (a) Make a purchase (of any amount) at a participating Sam's Club retail location between January 1, 2012 and March 31, 2012, be the recipient of a survey invitation on your cash register receipt, log onto the website on the top of your Sam's Club cash register receipt (www.survey.sams
