open to legal residents of the 50 United States and the District of Columbia, age 18 or older at the time of entry. Sweepstakes is invalid for patients whose prescription claims are reimbursed by: 1.) Any government program, including without limitation, Medicare, Medicaid, or any other federal or state program, or 2.) any third party payor in the state of Massachusetts.
begins at 9:00 a.m. on 1/1/12 and ends at 11:59 p.m. on 3/15/12
Limit of one entry per person or email address per day.
PRIZES: One (1) Grand Prize: $5,000 cash for winner's use towards a vacation to be booked through travel agency of Sponsor's choice; trip must be taken by 3/15/13. Ten (10) First Prizes: U.S. airline voucher ($200 Approximate Retail Value or "ARV" each). Total value of all prizes: $7,000.
