Sweepstakes begins at 12:01 AM Eastern Time ("ET") on 3/14/12 and ends at 11:59 PM ET on 4/30/12.
Open to legal residents of, and physically residing within, the United States including the District of Columbia (except residents of Kentucky), age 18 and older who possess an email address at time of entry. Void in Kentucky and where prohibited by law.
"Like" the Encore Dental Facebook page, enter through the Tooth Fairy Sweeps tab - https://www.facebook.co
Receive one (1) entry ("Primary Entry") upon completing the Sweepstakes Form, including responding to the Tooth Fairy Poll questions. After 'Liking' and entering via the Encore Dental Facebook brand page, you will have the opportunity to earn up to twenty (20) additional entries when your friends/relatives enter the Sweepstakes via the links shown on your Facebook wall or message after you post-to-wall or invite your friends.
Limit: one (1) Primary Entry and up to twenty (20) additional entries per person/email address for the duration of the Sweepstakes Entry Period.
Prize: One winner will receive one (1) $500 Visa Gift Card (retail value: $500.00).
