Sweepstakes begins on April 9, 2012 at 12:00 a.m. Eastern Time ("ET") and ends on May 31, 2012 at 11:59 p.m. ET.
Open only to legal residents of the fifty (50) United States (including District of Columbia) who are at least eighteen (18) years old at the time of entry.
There are three (3) ways to enter the Sweepstakes, as detailed below.
Free method (Online): During the Promotion Period, to enter for free without using a text-messaging capable device or opting-in to receive future communications from Sponsor, visit http://m.lasikplus.com/Eyewin.html.
Text Message: During the Promotion Period, using a two-way text-messaging capable device, text EYEWIN to 60600. Text EYEWIN STOP to 60600 to opt-out. Text EYEWIN HELP to 60600 for information.
Online (Opt-in): During the Promotion Period, visit http://www.lasikplus.c
Limit: Regardless of method of entry, each entrant may enter one (1) time during the Promotion Period. Multiple entrants are not permitted to share the same email address or text-messaging capable device.
Special Offer: After you have entered the Sweepstakes, you will receive a Special Offer coupon redeemable for 10% off a LASIK *Procedure (terms and conditions apply, *Procedure is for a traditional bilateral treatment: Acuity plan will be determined by surgeon. Procedure must be performed at a LasikPlus Vision Center by 12/31/12). Approximate Retail Value ("ARV"): $400. Special Offers will be fulfilled as a digital code by Sponsor via text message (for entrants who entered by text message) or via email (for entrants who entered Online). Entrants must present a confirmation text message or email to be eligible for discount. Special Offer is non-transferrable and cannot be combined with any other offer, discount or reward. Patients who have had any refractive surgery prior to entering into the sweeps are not eligible for this Special Offer. Enhancement procedures do not qualify for this Special Offer.
THREE (3) GRAND PRIZES (one (1) per Entry Period): A Certificate redeemable for a free LASIK *Procedure (terms and conditions apply, *Procedure is for a traditional bilateral treatment: Acuity plan will be determined by surgeon. Procedure must be performed at a LasikPlus Vision Center by 12/31/12. Certificate is transferable but candidacy must be preapproved by LasikPlus). ARV: $4,000.
THREE (3) FIRST PRIZES (one (1) per Entry Period): A $1,000 American Express gift card (terms and conditions apply). ARV: $1,000.
For All Prizes: Grand Prize is fully transferrable (only in its entirety, not as a partial procedure), First Prize is non-transferable, and no substitution will be made except as provided herein at the Sponsor's sole discretion. Limit: one (1) prize per person.

Administrator will randomly select the six (6) potential Sweepstakes winners, one (1) potential Grand and one (1) potential First Prize winner per Entry Period, on or around the dates outlined in the chart in Section 3 above, from all eligible entries received during that Entry Period. Non-winning entries from each Entry Period's drawing will not carry over into the subsequent Entry Period drawings.
