Sharecare Make A Change Sweepstakes ~ 8/31/12

Sweepstakes begins at 9:00 AM Eastern Time ("ET") on July 2, 2012 and ends at 8:59 AM ET on Aug. 13, 2012.

Open to legal U.S. residents (excluding residents in Puerto Rico, Guam, U.S. Virgin Islands, and other U.S. territories) who at time of entry are: at least eighteen (18) years of age.

TO ENTER: For purposes of these Official Rules, a "week" is defined as the time period beginning on Monday at 9:01 AM ET of the calendar week and ending Monday at 8:59 AM ET of the following week. See below for how to enter the Weekly Prize portion and Grand Prize portion of the Sweepstakes.

GRAND PRIZE: Each participant will automatically receive an entry into the Grand Prize drawing for each Weekly Prize drawing the participant enters.

One (1) entry per Weekly Prize drawing per email address.

WEEKLY PRIZE: Weekly Prize winners will receive the prizes in the table above. Total ARV of all weekly prizes is $9,053.50. Winners will be notified by Aug 31, 2012 if they have won. Winners will receive their prize within 6-8 weeks following verification of eligibility and confirmation as a winner of the Sweepstakes. Limit one (1) Weekly Prize per person.

GRAND PRIZE: Grand Prize winner will receive $6,000.00 cash. Total ARV of Grand Prize is $6,000.00. Winner will be notified Aug 31, 2012 if they have won. Winner of Weekly Prize can also win the Grand Prize.

TOTAL PRIZE VALUE: The total ARV of all Weekly Prizes and Grand Prizes is $15,053.50.
