Each sweepstakes (the "Promotion") is open only to legal residents of the 50 United States and the District of Columbia who are at least 18 years old as of the date of entry, except officers, directors, members and employees of the Sponsor, the judging organization (if applicable), or any other party associated with the development or administration of these Promotions, and the immediate family (i.e., parents, children, siblings, spouse), and persons residing in the same household, as such individuals. The Promotions are void outside the 50 United States and the District of Columbia and where prohibited.

3. HOW TO ENTER. There are two ways to enter: (1) Enter by using a smartphone, launch the Microsoft Tag app, scan a picture of the sweepstakes tag featured in the November 2012 issue of SELF Magazine, provide your information once as instructed on the Web page, and click to submit the entry form. This application is needed to scan the tag. Your carrier's messaging and standard carrier rates may apply. You must have a camera-enabled phone capable of sending pictures and accessing the Internet. 5. PRIZE DESCRIPTION. The number of prizes, prize descriptions and approximate retail value (ARV) of each prize for each Promotion is outlined below:

Cover and page 18: There will be one (1) winner. The prize is a two- (2) night, three- (3) day stay in an ocean-view room for two (2) adults (age 18 or older) at The Westin Beach Resort and Spa, Fort Lauderdale, Florida (ARV: $500); two (2) $200 airfare vouchers to be used on any airline (total ARV: $400). Vouchers may be subject to restrictions and/or expiration date. Dinner for two (2) at Shulas on the Beach up to $100 (ARV: $100); daily poolside cabana (ARV: $200). Trip must be booked by May 16, 2013, and completed by October 16, 2013, and may be subject to certain other restrictions, including blackout dates. The ARV of the prize package is $1,200, depending on exact itinerary. Actual value may vary based on fluctuations in price for accommodations. Ground transportation, meals (except for dinner), spa services, in-room or other personal charges at lodging (e.g., minibar, movies), telephone calls, gratuities, incidentals and all other expenses are not included. Winner and travel companion must be available to travel on the same itinerary. Sponsor will attempt to accommodate winner's preferred itinerary, but all specifics thereof will be at Sponsor's sole discretion. Travel companion must be 18 years of age or older as of the date of departure.
