Limit: You may enter one (1) time per day during the Promotion Period.

Prizes: There are two prize pools.
a. Veterinarian Hospital Employees: ONE (1) GRAND PRIZE: A trip for three (3) to
Chicago, IL. Trip package includes round trip, coach-class air transportation for
three (3) (the winner and two (2) companions) from a major airport near winner’s
home (determined by Sponsor in its sole discretion) to Chicago, IL; three (3) nights'
accommodations at a hotel determined by Sponsor in its sole discretion (single room,
triple occupancy); three (3) tickets to the AVMA 2013 Convention; and $500
spending money. Winner must travel on July 20, 2013 and return on July 23, 2013
or prize will be forfeited.

Pet Owners: TEN (10) PET OWNER PRIZES: A $100 gift card for an online retailer
(terms and conditions of gift cards apply). ARV: $100. Odds of winning a Pet Owner
prize depend on the number of eligible entries received during the Promotion Period
from pet owners. Limit: One (1) Pet Owner prize per person.

THREE (3) MONTHLY
PRIZES (one (1) per Monthly Entry Period): A $2,000 donation to an eligible 501c3
charity determined by the pet owner's nominated veterinarian hospital.

Limit: One (1) Monthly Prize per person.
