HOW TO ENTER: There are two (2) ways to obtain a UPC Code during the Sweepstakes Entry Period. Method #1: Visit any retailer where milk is sold, purchase Milk and look for the UPC Code. Method #2: To obtain a free UPC Code to enter the Sweepstakes without visiting a retailer or making a purchase, send a self-addressed stamped envelope to Breakfast Blitz Sweepstakes UPC Code Request, Post Office Box 9310, Medford, NY 11763-9310, postmarked by February 1, 2013, for receipt by February 8, 2013. (VT residents may omit return stamp.) Limit one (1) request per separately stamped envelope. During the time period between February 2, 2013, and February 17, 2013, in lieu of writing for a free UPC Code, you must send an e-mail to BreakfastBlitz@sweepspros.com to obtain a free UPC Code. The subject line in your e-mail request must state Breakfast Blitz UPC Code Request. You will receive the free UPC Code as a reply to your e-mail request within approximately one (1) business day. Each request for a UPC Code must be sent separately. After obtaining a UPC Code using Method #1 and/or Method #2, enter the Sweepstakes by visiting www.milkmustache.com to enter your UPC Code to register and follow the online instructions to submit your Sweepstakes Entry ("Entry"). There are five (5) Tiers in this Sweepstakes which are categorized based on the cumulative total number of Entries one has submitted. Your first Entry into the Sweepstakes will enter you into the Tier 1 Drawing for a chance to win one (1) of the twenty (20) Tier 1 Prizes. Entering the Sweepstakes with four (4) additional UPC Codes accumulating a total of five (5) Entries will enter you into the Tier 2 Drawing for a chance to win one (1) of the fifteen (15) Tier 2 Prizes. Entering the Sweepstakes with one (1) additional UPC Code accumulating a total of six (6) Entries will enter you into the Tier 3 Drawing for a chance to win one (1) of the four (4) Tier 3 Prizes. Entering the Sweepstakes with two (2) additional UPC Codes accumulating a total of eight (8) Entries will enter you into the Tier 4 Drawing for a chance to win one of the ten (10) Tier 4 Prizes. Entering the Sweepstakes with two (2) additional UPC codes accumulating a total of ten Entries will enter you into the Tier 5 Drawing for a chance to win the Tier 5 Prize. Entering the Sweepstakes with each additional UPC code after you have already accumulated ten (10) Entries will give an additional chance into the Tier 5 Drawing. Entries generated by script, macro or other automated means will be void. All Entries become the sole property of the Sponsor and will not be returned. Limit: one Entry per email address per day throughout the Sweepstakes Entry Period.

PRIZES AND THEIR APPROXIMATE RETAIL VALUES (“ARV”): One (1) Tier 5 Prize: A three (3) day / two (2) night trip for winner and three (3) guests (“Group”) to Super Bowl XLVIII in Rutherford, New Jersey (“City”) (“Event”) on Sponsor-selected dates tentatively scheduled for February 2014 (“Trip”). The Trip consists of the following elements for the Group: [a] hotel accommodations (one standard quad occupancy room, including room tax) for two (2) nights; [b] tickets to the Event; [c] hotel/Event transfers in New Jersey; [d] if winner’s residence is located more than two hundred fifty (250) miles from the City, round-trip coach air transportation on a Sponsor-selected carrier between a Sponsor-selected major metropolitan airport near winner’s residence (“airport of origin”) and City; or if winner’s residence is located less than two hundred fifty (250) miles from the City, round-trip ground transportation via a Sponsor-selected method between winners’ residence and the hotel; [e] airport/hotel transfers if applicable; ARV: $25,000.

Ten (10) Tier 4 Prizes: One (1) Year’s Supply of milk awarded as a check in the amount of $225.00 (ARV: $225.00 each). Four (4) Tier 3 Prizes: Two (2) tickets to a hometown game (ARV: $200.00 each).

If a Tier 3 winner does not have a hometown teams or there are no hometown games taking place within one (1) year from the time the winner is confirmed based on Sponsor’s discretion, s/he will receive a check in the amount of $200 in the corresponding winner’s name. Fifteen (15) Tier 2 Prizes: NFLShop.com Gift Card (ARV: $200.00 each). Twenty (20) Tier 1 Prizes: Breakfast Kick Off Kit with "got milk?" branded breakfast items (ARV: $75.00 each). Total ARV of all prizes: $32,550.00.

VOTING: The Sweepstakes also consists of a voting component whereby during registration, the user will be asked for a zip code. Upon entering the zip code, the user will see a list of schools within that zip code. Each entry into the Sweepstakes will earn one (1) vote for the school selected. The top 250 schools that receive the highest number of valid Votes will receive a $1,000 Fuel Up to Play 60 Grant to use towards school breakfast programs.
