The Face – ULTA Flyaway Sweepstakes (“Sweepstakes”) will begin on December 30, 2012 at 12:00 AM ET and end on January 19, 2013 at 11:59 PM ET (“Sweepstakes Period”). All times in the Sweepstakes refer to Eastern Time (“ET”).

ELIGIBILITY: Open only to permanent, legal United States (“U.S.”) residents who are physically residing in one (1) of the fifty (50) United States or the District of Columbia (excluding Puerto Rico, Guam, the Virgin Islands and other United States territories), and who are twenty-one (21) years of age or older as of the start of the Sweepstakes Period.

TO ENTER: To enter Sweepstakes during Sweepstakes Period, you must be a member of ULTA’s Reward Program (“Membership”). If you do not already have a Membership, go to https://www.ulta.com/ulta/myaccount/login.jsp and follow the provided instructions to complete the registration form to become a member of ULTA’s Rewards Program. You have to agree to the terms and conditions of Membership.

Limit five (5) Entries per day per person or email address during the Sweepstakes Period. Multiple Entries received from any person or email address beyond this limit will void all such additional Entries.

Sponsors will make two (2) attempts to notify potential Winner at the phone number and/or email address submitted at the time of Entry. Potential Winner may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Sweepstakes Documents”) within one (1) day of such notification.

PRIZE: There will be twenty (20) first prizes (each a “First Prize”), one (1) First Prize will be awarded to each First Prize Winner, and one (1) grand prize (“Grand Prize” and together with First Prizes may be referred to herein as “Prize” or “Prizes”) awarded as follows:

First Prize: Each First Prize consists of: one (1) ULTA gift card for one hundred dollars ($100) and (1) gift bag (“Gift Bag”). Each Gift Bag will consist of the following items: two (2) ULTA eye shadows; one (1) ULTA mascara; one (1) ULTA eyeliner; one (1) ULTA cheek color; one (1) ULTA baked bronzer; two (2) ULTA lipsticks; one (1) ULTA super shiny lip gloss; and two (2) ULTA nail polishes. The Estimated Retail Value (“ERV”) of each First Prize is two hundred and seventy five dollars ($275).

Grand Prize: One (1) Grand Prize will be awarded to Grand Prize Winner. Grand Prize consists of a trip (“Trip”) for Grand Prize Winner and one (1) guest (“Guest”) to New York, NY to attend The Face premiere party scheduled to take place on February 5, 2013 (“Event”), and one (1) ULTA gift card for one hundred dollars ($100). Trip includes: 1) round-trip coach-class airfare for Grand Prize Winner and Guest from a major airport near Grand Prize Winner’s residence (as determined by Sponsors in their sole discretion) to a major airport in or near New York, NY (“NY Airport”); 2) hotel accommodations for Grand Prize Winner and Guest for two (2) nights (standard room, double occupancy) in a hotel in or near New York, NY (“Hotel”); 3) round-trip transportation for Grand Prize Winner and Guest between NY Airport and Hotel; 4) general admission for Grand Prize Winner and Guest to Event; and 5) round-trip transportation for Grand Prize Winner and Guest between Event and Hotel. Trip must take place between February 4, 2013 and February 6, 2013. Guest must be twenty-one (21) years of age or older. ERV of Grand Prize is four thousand one hundred dollars ($4,100).

Grand Prize Winner must be able to travel February 4, 2013 through February 6, 2013 (or on such other dates designated by Sponsors). Trip awarded to Grand Prize Winner must be taken within dates provided by the Sponsors or the Prize will be forfeited.
