VSP Eye on the Prize 2013 Sweepstakes & IWG ~ 1/31/14

Sweepstakes begins on February 12, 2013 at 12:00 a.m. Pacific Time ("PT") and automatically ends on January 31, 2014 at 11:59 p.m. PT. Sweepstakes Period consists of ten (10) Entry Periods outlined in the chart below.

Open only to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry.

How to Enter: During the Sweepstakes Period, visit http://vsp.promo.eprize.com/envision2013 and follow the links and instructions to complete and submit the registration form, including a valid email address.

Upon registering, you automatically will receive one (1) entry into the applicable Entry Period drawing. If you register/login during one of the Game Periods, you will then receive one (1) Instant Win Game play for that day.

Limit: Each participant may enter only one (1) time per day during the Sweepstakes Period. Each participant may play the Instant Win Game one (1) time per day during the Game Periods. Multiple entrants are not permitted to share the same e-mail address.

Tell A Friend Program: After you enter the Sweepstakes, you will be invited to submit the names and e-mail addresses of up to five (5) friends (“persons”), who must each be eligible to enter the Promotion. For each referral of up to five (5) eligible friends, with valid e-mail addresses, you will be given one (1) additional entry into the Sweepstakes up to a total of five (5) additional entries per Entry Period. If a participant who registers for the Promotion through a referral email you sent is selected as a Sweepstakes Prize winner, you will be a potential winner of that same prize.

Non-winning entries from each Entry Period will not carry over into subsequent Entry Period drawings.

The Instant Win Game consists of three (3) Game Periods and the beginning and end dates correspond with Weekly Entry Periods 3, 7, and 10 in the chart above.

One (1) Entry Period 5 Prize: One (1) Bose SoundDock 10 bluetooth digital music system. ARV: $600.

One (1) Entry Period 6 Prize: One (1) Kindle Fire HD. ARV: $200.

One (1) Entry Period 7 Prize: One (1) 16GB iPad Mini. ARV: $329.

One (1) Entry Period 8 Prize: One (1) Cannon Powershot Camera. ARV: $450.

One (1) Entry Period 9 Prize: One (1) Vizio 60" LED TV. ARV: $1,000.

One (1) Entry Period 10 Prize: One (1) 16GB iPad. ARV: $499.

Tell A Friend Prizes: If a Sweepstakes Prize winner registered for the Promotion via the Tell a Friend Program described above, the entrant who referred the Sweepstakes Prize winner will win an identical prize as the Sweepstakes Prize winner. If the Sweepstakes Prize winner did not register for the Promotion via the Tell a Friend Program, then the corresponding prize below will not be awarded.

Instant Win Game Prizes:

Thirty (30) Online Retailer Prizes (Ten (10) per Game Period): A $25 gift card to an online retailer. Approximate Retail Value (“ARV”): $25.

Thirty (30) iTunes Prizes (Ten (10) per Game Period): A $25 iTunes gift card. ARV: $25.

Thirty One (31) Starbucks Prizes (Ten (10) for Game Periods 1 and 2 and eleven (11) for Game Period 3): A $25 Starbucks gift card. Approximate Retail Value (“ARV”): $25.

Limit: one (1) Sweepstakes and one (1) Instant Win Game prize per person. Total ARV of all prizes: Up to $11,395.
