There are 2 ways to obtain a sticker to play:
Purchase a 30 fl oz or 42 fl oz Coca-Cola® beverage at a participating Church's Chicken Restaurant and you will receive a cup with an Instant Win Game sticker
Or
Hand print your name, home mailing address, day and evening phone numbers, email address (if any), and date of birth on a 3" x 5" piece of paper along with a self-addressed stamped envelope (legal residents of Vermont do not need to include a self-addressed stamped envelope) and mail it in an envelope with proper postage to "Church's Chicken Family Reunion Peel and Win - Free Sticker," c/o ePrize, Inc., P.O. Box 5046, Kalamazoo MI 49003-5046, Department 596379, to request a Sticker. Limit: 1 request per envelope, up to 49 total requests per person.
Once you have the Sticker, peel off the Sticker to reveal a unique code. Use the unique code during the Instant Win Game Period or any time before August 14, 2013, and follow the instructions to see if you are an instant winner
