I got mine from throughway authority (off 287 in rockland county) 14 years ago, and never paid a monthly fee. I have 3 tags. So it appears it depends on where you get your tag.

I use an I-Pass (Illinois) which works everywhere ezpass does, it has (I assume still has) no fee, the only thing I paid was a $10 deposit which you get back when you return it.
