Usually amazon sticks MIR form to their product page, but this one is not. I suspect they may accept MIR only for cash register receipts as form says in it.

Well, that's what I read too. No cutting and pasting the UPC label from the box, just write the UPC Code and send the original receipt with the rebate form. There are 2 less spaces for the UPC Code...
