You should only need to connect the DVR to the home theater receiver via HDMI (input) and then the receiver to the TV via HDMI (output). The other audio cables from the DVR to the receiver aren't...

The easiest way to connect a device like this is:
Attach it to the TV with an HDMI cable.
Attach a TOSLINK cable between the TV's audio out and the home theater's audio in.
Attach your directv...

With that setup, the HTIB (Home Theater in a Box) needs to support HDMI pass through while off (which I do not believe it does) or else the HTIB must be on all the time to get sound from the DVR.
