You keep what's in your backpack. If you throw stuff on the ground it's now part of the server. And yes, you spawn in the same spot.

the launcher just updated the game for me today and it works fine now.
Make sure your Arma 2 version is 1.11 and CO is 1.60. Also, try running DayZ Commander and clicking on "Versions" in the upper right, this will tell you if your patches are up to date.
