Total should be $250, not $2500. So averages just under $17/visit. Technically, if you spend $241 at one place, you can spend $1 on the other 9 visits and still qualify.

No, I think you get 1,500 for each $250 you spend. To get 15,000 points, you'd have to cross the $250 threshold 10 times (so at least $2,500).

That's not how I read it. $250 total spend no matter how many visits nets you 1,500 miles. You can get another 1,500 by spending $250 more. That means to get the maximum 15,000 miles you need to...

Yes and yes. Remember, you have to spread it out across 10 visits. With Sep 30 to complete the offer shouldn't be too difficult. Each visit will get you 1500 miles, once you complete the survey...
