I simply saved the page with the rebate form as HTML. Opened it in Microsoft Word and make sure ALL fonts are BLACK. Saved and closed the file. Opened it again in the browser and everything...

I mailed the rebate the day after the slippers were delivered. The rejection letter arrived last Monday or Tuesday. Obviously I'm going to fight it... already replied to them with a letter. :)

For a clear and easier to read rebate form, I used firefox to highlight the text of the rebate. Then print "selection" and in landscape mode. Prints out larger and easier to read.
