If paying with prepaid refill phone cards, it would be $30 flat. No tax, no fees. (No tax when buying phone cards from the stores, either.) Not sure about paying with credit cards.

The Activation Card that came with my microSIM no longer says "Initial $3.34 value" on it anymore like it used to in the past. Maybe that's just because they list other prepaid options now besides...

$100 will get you 1000 minutes that will expire in 1 year. Do this, when you buy the sim, it will come with a starter package of 10 minutes after you have activated the card and chose a phone...

In addition, T-Mobile uses the 1900 band for 2G and 1700/2100/AWS band for 3G. If your phone was not from T-Mobile, it is likely to support the first but not the second. 3G is relevant for data...
