Make sure you get proper receipt. See below for what is required. Sometimes you may not get an acceptable receipt with your order. If not call or email to get a good copy.
The invoice/receipt MUST have:

You need two UPCs to claim the rebate. DO NOT peel the top layer with the UPCs, cut them out leaving a hole in the box with all layers of cardboard intact.

Basically you may need pictures of the serial numbers from the camera and printer in the event your UPC get lost at some point in the process. Copies of the UPC are apparently not sufficient. You may want to take pictures if you plan on selling the items before getting the rebate.
Mailing the receipt
I've done numerous of these deals and other deals with rebates by just using first class stamps and have never had a problem. If you do want to do it that way check the weight as it might be over what one stamp will cover.
Some people for more at ease with having tracking to prove delivery of their submission.
If you would like to track your rebate and have a Paypal account. You can use the Paypal Ship Now link to send it First Class with tracking for $2.66. It's the cheapest option you will find with tracking.

- UPC from the Printer box (Found on the outside of the printer above the serial number)( You can leave the serial number just the UPC is needed.)
- UPC from camera box (The UPC on the outside of the box under the picture of the camera. Do not use any secondary UPC that me be on the box flaps or elsewhere)
You have two ways you can file the rebate.
- Download the rebate form. Follow the directions, fill it out and attach the other required documents and mail it in.
- You have the option to Pre-submit the rebate online and print it for mailing with your required documentation. Go to http://rebates.usa.can
on.com or https://www.acbpromotio ns.com/canonrebates/ and click on the "Pre-submit your Rebate Claim" button to pre-submit your claim.
If you use presubmit option you do not need to use or mail in the form linked in the OP. Follow the direction on their web page.
IF Presubmitting
- the Offer Number is 8191462 which isfound in the address of the linked paper rebate form.
- The Primary Product is the Printer. The Pro-100
- The serial number for the printer is right below the UPC on the outside of the box, no need to open the box.
- The retailer is Focus Camera.
- After filling in the offer #, primary product and serial number carefully scroll down to you find the correct offer. That depends on what bundle you purchased.
- A form will be generated at the end. You will print & submit that form with the UPCs from the products & a copy of your receipt. You have to print & mail the form created and your supporting docs to get the rebate.
- Attach a copy of your receipt to the generated form. It must show the retailers name, order date and have the products listed separately with the date of purchase and the purchase price circled.
- Attach the original UPCs along.Do not peel the top layer with the UPCs off of the cardboard. They want a hole cut in the box and all layers of cardboard intact on the UPC.
- Mail the completed form along with the UPC and receipt copy to the address on the generated form(See below for tips on mailing)

My rebate got "lost" in the mail, and when I contacted them to ask if I could I send scan copies I made of the forms + upc cutouts they said I couldn't, and instead wanted a picture of serial number of the camera and inside the printer. Luckily I still had the camera (which I kept), but I already sold the printer. I had to contact the buyer across state and plead he take a picture of the printer inside so I could get my rebate
