What you should have done (and could do in 30 days if you wish to reapply) is to simply apply as a "Sole Proprietor". From that point, pick a category/subcategory that makes sense for you, put minimal levels of income and years in business. I have gotten 3 Chase business cards doing exactly that (2 using my SSN, the 3rd with an EIN), as have a large number of other people - oddly enough, it is easier to get approved following those steps than for legitimate small business owners applying for their businesses.
One thing to know, the buyer protections granted to customers of non-business VISA cards may not come with the business card. You may want to check carefully before using this card for higher risk purchases.

I made up a business name the first time and they asked for business license and proof of my business at the address. At the end it was denied.
Does anyone else have experience with this? I'll kill myself if I miss the $500 bonus

Applied on July 14th for the ink cash and got the wait 30 day response. Called on 7/19 to see if I could get auto approved but the machine said the following:
I have more than 1 app in the past 30 days. They received my FIRST application on July 14th which is currently being reviewed. I'll hear back in 7-10 business days.
They received my SECOND on July 10th and I'll hear back in 2 weeks.
The dates are flipped around... why is that?
Also got a letter in the mail a few days ago for the first application saying that I was denied for the sapphire. I thought I was good to go on applying for the second card.
Is there a minimum number I'm supposed to be putting down when answering the questions?

I don't know for sure but in general I believe if you have any sort of business even if that is just selling a few things for a few bucks here and there most credit cards for a business card without any forms. I believe the way you do this is by pudding either the name where the type of business as sole proprietorship. I don't know the exact details of how this works but if you do a Google search with that term Plus credit cards business card you should be able to find that information
