What you should have done (and could do in 30 days if you wish to reapply) is to simply apply as a "Sole Proprietor". From that point, pick a category/subcategory that makes sense for you, put minimal levels of income and years in business. I have gotten 3 Chase business cards doing exactly that (2 using my SSN, the 3rd with an EIN), as have a large number of other people - oddly enough, it is easier to get approved following those steps than for legitimate small business owners applying for their businesses.
One thing to know, the buyer protections granted to customers of non-business VISA cards may not come with the business card. You may want to check carefully before using this card for higher risk purchases.

There is a different portal for business cards. Called in to chase and they helped me to sign up for the "chase for business" and they were able to link my personal and business account in that chase for business portal

Details: I own an LLC of which I am the sole owner with high gross yearly sales. My husband and I file personal taxes jointly (he is listed first on the tax return)
I filled the form out and provided both my ss# and the FEIN for the business (as requested on the form). Should I have used his SS# since he is listed first on our tax return?
Also, category is sporting goods with the breakdown of sporting goods retail. Should I have chose something else?
I listed # of employees as 1 (myself). Should I have listed "0"?
We do not currently have any credit cards.
In business for 7 years, verifiable w tax returns.
Any helpful info would be much appreciated as I am at a point in business where I could really use this. Thanks in advance.

Yes, I just got my card in the mail today. Applied last week. First time that has ever happened to me!
