Amazon appears to be HD only
No, it only ports as high as the quality of the original purchase.
EDIT: Additionally, iTunes only upgrades purchases made in iTunes. If a title is upgraded to 4k at the time of purchase, it will port out at 4k where available, unless bought in a bundle or as a gift. If it isn't 4k on iTunes when you purchase, but later gets upgraded, then it will only port HD.

2 ways to get if it's showing HD in iTunes but 4k is available: redeem a code for it, or gift it to yourself, but if you gift it, the 4k stays in iTunes and doesn't port out. Redeeming a code is the best way.
