pilot holes, ALWAYS pilot holes for lags...
you should be able to drive lags into 2x material with a ratchet, by hand, if you've drilled the proper size pilot hole. if you're just driving them in blind with an impact, you're hurting the wood and compromising the integrity of your joist.

I will take solace in the fact that they have probably sold hundreds of thousands of these things and if there was a flaw in the design, we'd have all heard about it by now.

Other that that negative stuff most of the installers actually suggested upgrading the lag screws that hold the four brackets onto the ceiling. I went to home depot on bought some fancy new lag bolts to easy my mind. I never even thought about the four bolts you guys are talking about.
A Couple of other points.
The measurements that were talking about before, the length I just marked where I Wanted it and centered the top braces on those marks, you basically want them centered where the legs are going to drop down to support the deck. Then I just measured over 48 1/2 or whatever to the other side.
I put ours over our garage door, you actually loose the width(2in) of the L deck so think about that when deciding. I thought the deck sat at the bottom and not at the top of the deck. I don't know if that makes sense.
Lastly I had to angle my deck for whatever reason our garage doesn't sit horizontal when its fully opened.
Its not difficult but definitely have a friend help.

Also more tips, rub soap on your lags. It makes it easier to drive them into the joists and a 1/4 drive impact helps.

So if you mounted it at a 13 foot ceiling, the rack itself would be at a little over 9 feet. Still need a ladder.
My garage is similar....no finished ceiling since its detached from the house. I think my joists are probably 11' or so off the ground, I would probably still need a ladder even with the 45" drop.

the real concern is the bolts going straight up, vertically, into joists and more so... did you find the center of the 2x material? did you drive the lag straight? did you go through a weak point (knot or similar)? is the joist rotted somewhat? did you drill a proper pilot hole and not just drive a 1/4 bolt into 2x material and potentially stressing/weakening it?
aside from that... who knows what shit steel they used for the mounting brackets. but, by far, it's the bolts going into the joists that are the concern.
