i was selected for the note 9 study but ultimately declined to participate. i just received a similar short survey (basic contact and current phone info).

I passed on the note 9 because of the discount instead of free phone. Not a fan of being obligated to the phone and have to share data with them for a free phone it's a deal

Hummm.. interesting. I don't think it should matter.
But I just kept it simple by using the same email.
